My N.H.S. Number; OHBCD 297. D.O.B. 10.12.67. Registered at Highlands Medical Centre. Fareham. Subject. Covid 19 vaccine REQUEST. Dear Doctor, I am requesting that I be invited for a vaccine. I am an agency driver, driving for Linets, 11, Brunel Way, Fareham. PO15 5TX ; medical suppliers to the Hospitals widely. I have been transporting medical, blow up mattresses to various hospitals. Yesterday, I transported back to Linets, contaminated Covid 19, mattresses that were double sealed in plastic sacks and marked BIOHAZARD. The shop floor staff have had their vaccine. Therefore, I would like to be invited for a vaccine, ahead of time? I hope you can help me out in these busy times? I wait for a reply soon? Yours sincerely, Mr. R. John Newell.
