Gray Healthcare has created a bespoke service to manage and support individuals with a mental disorder or ABI in their own home. Positive risk taking enables patients in secure environments to step down sooner, with the support of an experienced wrap-around clinical team to manage risk professionally. The service recognises complex needs and challenging behaviours, therefore each service user is allocated up to 24 hours of 1:1 or 2:1 care a day dependent on their individual needs and risk. A tenancy in the service users name (sourced with the support from the Gray Healthcare Property Team), with Gray Healthcare acting as guarantor, provides stability and reduces need for future moves.

Gray Healthcare offers highly individualised and tailored care packages for service users who do not need the physical security of an inpatient facility, but do require ongoing treatment and support from an experienced clinical team, thus reducing the likelihood of relapse. Such packages will help bridge the gap for patients who require the least restrictive environment and offer a smooth transition into the community.

The bespoke care package includes qualified support staff (selected to match the service users needs), nursing, psychology and occupational therapy within a framework of risk management strategies in partnership with an NHS consultant psychiatrist (who will be the service users Responsible Clinician whilst in the community). The service provides rehabilitation in the individual's chosen home destination. The right input in the right environment produces results in both clinical and lifestyle recovery. Service users will be referred from a range of inpatient facilities, therefore freeing beds in line with government targets.
