We provide a person centred support services to Adult living with Dementia, Learning & Physical Disabilities.

Our services focus on supporting individual to live independently as possible in their home, support to regain independent skills, support to identify and take part in meaningful activities. We provide a tailor made support package to meet each clients needs. 
Our carefully selected Personal Assistant are experienced with adeqaute  knowledge and on-going training to provide quality support to  Adults with mild and complex need in the community. At Xtreme care we understand that everyone likes different things, therefore we make sure that our services are personalised to suit each individual needs.

24 hours client support
Promotion of independent living skills
Support for the client to identify and take part in meaningful activities
Supervising to take prescrided medication by GP
Organise introduction of suitable staff to client
Assessment of clients support needs.
Complete checks and DBS on all staff to standard
Monthly progress report.
Support to create a personalised Support plan
